- One-on-one in-home assessment and treatment by a Registered Physiotherapist

Who refers
Partners we work with
Referral is not required for a patient to access physiotherapy in Ontario, but coordinated referrals help ensure continuity after discharge.
Physicians & nurse practitioners
Patients whose mobility, balance or falls risk is limiting independence but who struggle to attend an outpatient clinic.
Hospital discharge planners
Patients going home after stroke, joint replacement, deconditioning or a prolonged admission.
Retirement homes & long-term care
Residents needing individualized one-on-one physiotherapy in their own room or suite, where the home permits outside providers and required permissions are in place. Intended to complement, not replace, existing facility or community rehabilitation.
Home care agencies & case managers
Clients requiring one-on-one rehabilitation to complement personal support or nursing services.
Stroke rehabilitation at home
Stroke rehabilitation is a core area of practice at PhysioHelps. Home-based functional rehabilitation can support patients transitioning from acute care or inpatient rehabilitation, and those for whom outpatient attendance is a barrier. This service complements, and does not replace, existing hospital, inpatient or community programs. No outcomes are guaranteed and no contractual commitment is implied.

Clinical presentations well suited to home care
- Stroke survivors continuing rehabilitation after discharge
- Older adults with recurrent falls or near-falls
- Deconditioning following hospitalization or illness
- Post hip or knee replacement recovery at home
- Parkinson's disease, MS and other neurological conditions
- Patients for whom travel to a clinic is a significant barrier
- Retirement home or long-term care residents, where the home permits outside providers
- Frailty, arthritis-related mobility limits or dementia-related mobility decline
What to expect
How a referral proceeds
- Contact by phone or email with the patient's consent
- Scheduling confirmed directly with the patient or family
- Comprehensive in-home assessment of function and environment
- Individualized plan with goals set alongside the patient
- Ongoing reassessment as ability changes
- Communication back to the referring provider on request and with consent

PhysioHelps is a mobile, service-area practice — treatment happens in your home, retirement residence or supportive living suite. There is no clinic to travel to. Visits cover Kitchener, Waterloo and Cambridge as well as New Hamburg, Baden, Shakespeare, St. Marys, Seaforth and nearby communities.
